I agree to have eyelash extensions applied to my natural eyelashes and/or removed. By typing my name and information in the fields below, I consent to the placement and/or removal of the eyelash extensions provided by the certified lash specialist. 

I understand the risk associated with the application and/or removal of eyelash extensions.

I understand that in rare occasions there are risk associated with having artificial eyelashes. I further understand that in rare circumstances eye or skin irritation may occur such as redness, watery eyes, or irritation.

I understand that because of the natural lash cycle, my lashes will eventually shed. I will need to maintain my touch up appointments at least 2-3 weeks after my appointment to maintain a full look.

I understand and agree to the aftercare instructions and for any unexpected circumstance(s) that can happen due to not following these instructions are at my own risk.

I understand that I will need to remove my contacts, if any, before every appointment.
