*A CONSENT FORM WILL NEED TO BE FILLED OUT FOR ALL NEW CLIENTS WHEN THEY ARRIVE TO THEIR FIRST APPOINTMENT*

• Please note that I will NOT rush on any sets. So please do not over lap your booking.

• I only open my book one month at a time, so do not book a month in advance (unless it’s the 22nd of the current month or after) or your appointment will automatically be CANCELED

• I do NOT go over others work.

• Lashes must be cleaned & free of glue/makeup before arrival.

• Address is given the day before your appointment.

• Remaining balance due in CASH ONLY!

• All deposits are NON-REFUNDABLE & TRANFERABLE ONE TIME WITHIN SAME MONTH.

• Your deposit does go toward your service.

• NO CALL NO SHOW=BLOCKED FROM BOOKING

• No extra guest unless being serviced - NO CHILDREN!

• Provide accurate contact info so I am able to reach you.

• If you do not confirm your appointment with me AT LEAST 2 HOURS BEFORE your appointment, it will be cancelled

• 15 minutes late = Cancellation

• 10 minutes late = $10 Late Fee

• Same day bookings require an additional $30 due before appointment is confirmed.

• Squeeze in appointments require an additional $30 as well before confirming appointment. A squeeze in is any appointment that I have to put in myself. That incudes days that are not in my normal business hours.

• If you need to CHANGE/RESCHEDULE your appointment, you must text (412) 900-5227. Keep in mind that if you already rescheduled, you will need to send another deposit. 

Please arrive to your  appointment with a mask. If you are feeling sick do not book an appointment. Any frequent coughing, sneezing etc. counts. We want everyone to be safe.
